An advanced practice nurse collaborates and communicates with patients, families, doctors, nurses, and specialists to ensure patients receive the care they need. As they diagnose, treat, manage, and educate patients, they are responsible for ensuring patient safety and maintaining ethical behavior. Competencies have been developed to help the advanced practice nurse to understand the practice knowledge, skills, and attitudes they need to be successful.
For this Discussion, you will examine advanced nursing practice competencies and reflect on your strengths and challenges related to the competencies. In light of your reflection, you will consider how this course may help you attain your career goals or objectives.

Main Discussion Post – CAREER GOALS: STRENGTHS AND CHALLENGES RELATED TO NURSING PRACTICE COMPETENCIES
Due to the fact that family nurse practitioners care for their patients over their lifetime, the nurse practitioner programs are rigorous, which presents the student with challenges and obstacles are to face while completing these program (McNelis et al., 2021). I have set many goals for myself throughout this program and my main goal is to prove to myself and my children that if you set your mind to something, no goal is too far out of reach. I expect throughout my time in the course to be able to analyze my professional growth and development as it related to the advanced nursing practice, examine conditions of the body systems of adult patients, develop the skills to formulate a diagnosis and treatment options for these patients, and develop the ability to provide competent care for an adult through their lifespan.
One of my greatest strengths is that most of my nursing career has been spent caring for adult patients. Taking care of adults is comfortable for me and I can relate and talk to this population. Another strength I possess is that I have cared for a wide range of patients during their hospital stay from inpatient rehab, at the end of their stay, to the intensive care unit during the most crucial time of their hospitalization. The experiences I’ve had as a nurse has allowed me to feel competent and confident in assessment, monitoring, and communicating with members of a team to care for these individuals.

Some of my greatest challenges in this course will be working with patients over their lifespan and following up on care. In my career, I have only worked with patients in the acute care setting. I have not had many opportunities to be able to follow up on an individual that has been discharged from the hospital and see how their current health status is. One of the major goals I have in this class is to feel more confident caring for patients over their lifespan. One objective that I hope to familiarize myself with is helping patients make lifestyle changes to improve their health and help prevent chronic illnesses from occurring. While in a primary care setting, it is important to not only educate patients on lifestyle changes they can make to better improve their health but offer them resources to help meet their goals. Many chronic diseases occur from patient nutritional and lifestyle statuses however, in the clinical practice it is rarely implemented (Chao et al., 2021). One obstacle that I would like to achieve is to help incorporate education on the importance of these lifestyle changes and support patients by offering the patients the tools and resources they need to live a healthier lifestyle to improve overall health. Offering patients nutritional and lifestyle changes education can help prevent chronic conditions such as cardiovascular disease, diabetes, and cancer (Chao et al., 2021). I hope to also find ways to motivate patients to want to improve their overall health and follow up on the strategies implemented. Another challenge for me is to learn barriers that patients face with access to healthcare and achieving health goals. In the hospital, patients are taken care of no matter their access to healthcare, but when patients are discharged, what barriers are faced and how can we help to eliminate these barriers? Chronic illnesses are the leading cause of premature mortality, disability, and morbidity (Morris et al., 2022). Chronic conditions also lead to a decreased quality of life, increases the need for health care services which in turn ends up inflating health care costs (Morris et al., 2022) What modifiable lifestyle changes can we as providers offer to these patients to help prevent these from occurring? Is the barrier a lack of education from providers or how the education is being provided? I hope that throughout the time in this class, I can fully have a better understanding of the outpatient world and what I can do as a provider to offer support to these individuals.

Your thoughtful analysis highlights how difficult and rewarding it is to become a family nurse practitioner (FNP). Your desire to learn a comprehensive method of patient care throughout your lifetime is admirable and fits in nicely with McNelis et al.’s (2021) claim regarding the demands of FNP programs.
Given your expertise in acute care settings, I recommend concentrating on long-term patient relationships and preventive care. Investigating motivational interviewing methods is one strategy. My expectation for this course is to gain knowledge and skills to make proper diagnoses and clinically manage healthcare needs for adults across the lifespan (Walden University, n.d.). In addition, I wish to build my Nurse Practitioner Core competencies to prepare myself for the ever-changing healthcare challenges and bring forth positive outcomes for my clients and health organizations (The National Organization of Nurse Practitioner Faculties, 2012).
As a nurse, I had the opportunity to work in different work environments: an acute adult psychiatric facility, a CV-ICCU unit at the local hospital, and a clinic-type setting at an adult state hospital. My strength is the knowledge and skills I have gained through these experiences and the challenges I have faced in patient care and working with upper management. Unfortunately, this is also my weakness. I have never worked with patients who were 17 or younger. A Family Nurse Practitioner (FNP) in a clinic-type setting will provide healthcare services to all age groups (AANP, 2019). I must gain the skills and knowledge to work with the younger population.
My career goal is to provide healthcare services to underserved populations, such as those living in the jungles of Laos and Thailand, my parent’s homeland. I can become a better practitioner and make cognitive decisions for managing health concerns for all age groups by gaining knowledge, skills, and experience.

Thank you for sharing such an informative discussion post. I agree my expectations are very similar to yours. I hope to gain the amount of knowledge to safely provide the best care to my future patients. While accepting my responsibilities, I can also carry out research to improve my knowledge of my practice in these areas. I will be able to retrieve evidence-based information that I can utilize in my quest to achieve my clinical and professional goals and objectives (Wei et al., 2019). Conducting research is an opportunity to advance my knowledge and skills in various areas using the best available literature. I think the best thing about advancing your education and becoming an advanced healthcare provider is that we all are unique and have different expertise and skills in certain areas. Our weaknesses are the opposite as I am worried that the experience I have gained over years in working in a specialized area such as Labor and Delivery and NICU will hinder my career. I am more comfortable with pediatric patients compared to adults or psychiatric patients.
Providing education to the patient, a collaborative attitude and good communication are vital for patient care. Collaboration with other healthcare professionals will open the channels of best practice possibilities (Haddad 2021). In addition, cooperation in clinical practice improved the quality of patient care as professionals shared expertise. As we start with our practicum, there will always be challenges met, but preparation is vital to gain more confidence and overcome the fear.

I enjoyed reading your post and the way you clarified that gaining the necessary knowledge and skills to properly diagnose patients will help us improve our competency as we enter the workforce. It’s not just the class, the Walden culture of building the skills and confidence of the students to deliver quality care while making a life-changing impact for the patients and communities we serve is great inspiration to want to be better. The increased access to health care and reduced costs have warranted the high utilization of Nurse Practitioners (NPs), therefore well-qualified and highly competent NPs may positively contribute to access and utilization of primary care services (Ortiz et al., 2018).
You mentioned your great desire to gain knowledge and skills to work with all age groups, especially patients under 17. Thank you for your post, you made me realize my weaknesses in working with teenagers. This clinical will help us get the needed hands-on experience, and we might have to be intentional about getting more of the age groups we need to gain more confidence around when working with preceptors. I think since you have had challenges working with management it is going to be great for you in your new role since you will be a leader yourself. Our experiences shape the kind of providers we will be in so many ways. Looking at improving leadership as an NP, you can utilize the American Association of Nurse Practitioners as a resource. Since for NPs, leadership occurs within the organizations, networking with other leaders and building bridges is key to strengthening our collective professional and clinical excellence (Fernandez, 2022). You mentioned a compelling career goal of providing healthcare services to underserved populations in Laos and Thailand. That will be a great experience and I believe the thorough assessment skills, and critical thinking we are acquiring will help improve the knowledge and skills needed to directly contribute to your ability to serve these communities effectively.

Nurse Practitioners should have core competencies in analyzing data and using evidence to improve the delivery of healthcare (The National Organization of Nurse Practitioner Faculties, 2012). When it comes to nursing care competencies, my strength falls in using evidence-based practice to advance the quality of healthcare we provide for our patients. Quality competencies are crucial for promoting the best clinical practice. Leadership competencies can be a challenge in nursing practice but are an essential part of becoming an advanced practice nurse (APN). This involves decision-making and problem-solving skills that contribute to patient outcomes. APNs are responsible for diagnosing patients which involves critical thinking, differential diagnosis, and integrating and interpreting data (American Association of Colleges of Nursing, 2010).
My career goal and objective is to become a confident and competent APN. This course will help me accomplish this goal by developing the critical thinking skills needed to diagnose, treat, and manage patients in the adult population. The course assignments will focus on disease processes that will assist in diagnosing acute and chronic conditions commonly found in primary healthcare. Identifying health problems, making diagnoses, and planning appropriate care is made possible by obtaining a thorough history and physical exam of the patient (Seidel, et al., 2011). Being able to record, organize, and synthesize this information is part of becoming a competent APN.

Wania, thank you for your informative post. Decision-making and problem-solving skills are critical to becoming a safe and exceptional practitioner. Our prior courses have prepared us to include what we learned, incorporating research into making an evidence-based diagnosis and plan for our patients. Developing the skills of incorporating evidence-based practice into patient care improves positive outcomes, up-to-date research supported, and cost-efficient care (Melnyk, 2018). I have learned that in practice, only some patients present with precise diagnoses. Learning to rule out other causes and ensuring that the list of complaints is not causing a life-threatening condition may lead to a treatment plan and relieving conditions. Often, a patient treatment plan must be individualized due to a patient’s ability or desire to follow such a plan. Using a patient-centered collaboration when developing a treatment plan will increase the likelihood that it will be followed (Ahmady & Shahbazi, 2020). When patients approach us with numerous, vague complaints, being able to take a history, assessment, and physical exam will help to come up with a treatment plan. These skills take practice.

High quality care encompasses using critical thinking, evidence-based practice, and clinical expertise when diagnosing, treating, managing conditions, and educating patients. It is essential for nurse practitioners to develop a relationship with their patients based on trust and adhering to standards of practice that lead to positive patient outcomes. Nurse practitioners are a vital part of the interdisciplinary team where collaboration amongst other providers and the patient takes place to development the proper plan of care.
This course will set a foundation for putting into practice the knowledge that I have gained thus far in the FNP program and the past clinical experiences that I have as a registered nurse. My expectations are that my mind set will shift since I am the provider who will be diagnosing and treating my patients. As a staff nurse, I have collaborated with the provider when it comes to the patient condition and recommended what treatment would benefit my patient however I was not the provider entering the necessary orders and diagnosing the patient. As a nurse practitioner student, I expect that I will utilize the tools I have been continually developing to provide care as an independent clinician.
After reviewing the NP Core Competencies, I am able to identify where my strengths and challenges are. Being able to identify these will contribute to my motivation to learn and grow as a medical professional who delivers care across the healthcare delivery system. Scientific Foundation Competencies include the use of research in order to improve practice and outcomes, develop new practice approaches, and participates in professional organizations that impact advance practice nursing. I have been on committees within my hospital organizations where I have been part of an interdisciplinary team that comes together to evaluate the hospital processes and improve where needed. My strength is that I am able to communicate effectively with all levels of staff within the organization. I have confidence that I am a leader who is motivated to improve health outcomes necessary within the workplace. I have experience acting as a leader which will help me when it pertains to the NP leadership competencies. I am able to collaborate with an interdisciplinary team with the intent to improve health care using evidence-based practice which also coincides with NP Quality Competencies. I believe that it is crucial to have leaders who are providing direct care to be a part of the collaboration process in improving practice. I believe this is where the greatest and most impactful change is made because it involves the voices of the leaders who are directing interacting with the patients. It is difficult to create necessary change if the team who is creating change includes those not interacting and providing care to patients. Practice Inquiry Competencies include using clinical practice to improve practice and patient outcomes. I believe that having many years of clinical experience while adapting to the continuous innovative changes within healthcare is a strength that I have because I am able to use these experiences when I am on committees to improve organizational change. My experience has helped me to develop clinical skills while still being adaptable to learning new skills. As a nurse practitioner I will learn a different skill set that will provide different experiences than I have had before. I am open to learning every day and learning from those around me. I am able to understand policies set in place which is vital to adhere to legal and ethical guidelines which is part of the Policy Competencies. I feel that my strengths include being culturally competent to others backgrounds which helps me to develop relationships with my patients. I am able to provide compassion and display confidence in my clinical capabilities. I feel that being open minded to other diversities helps me to work with my patients to develop the plan of care that is most suitable for them with the best outcome. I have the utmost respect for my colleagues and patients.
While it is important to identify where my strengths are, it is equally important to identify where my challenges are. When we are able to identify challenges that we have, we are able to work through them more effectively. I know that a challenge that I will have will be when it comes to the financial aspect as a nurse practitioner. Understanding how to bill services is something that seems complicated because it involves coding. Carolyn Bupport, 2020, explains the rules to billing services which has helped me a bit in understanding the financial aspect of how NPs bill for services. Veenema et. al, 2022, provides an analysis showing that “the type of clinician, state practice authority policy, number of NPs, and percent of population 65 yrs and older an population size are positive predictors for services billed.”
These are components that I didn’t have to involve in my care thus far in my career. Having to understand how insurance works will have to be something that I need to work at in order to be compensated properly for my care. Another challenge I have is that I am not tech savvy, but I am excited when there are new advances in technology within the healthcare system. I am open to learning which is necessary because more advances will be taking place as time goes on. Being able to keep up with these changes will be beneficial for myself and my patients.
This class will help with my career goals in many ways. The class will help me set a foundation to be an independent practitioner who is able to competent to deliver high quality care for my patients. My career goals include being competent in the NP Core Competencies. The competencies will also help as a guide to self-reflect on where I have strengths and where I need to gain strengths.

Thank you for sharing your thoughts. I also feel the same about our educational experience, which has taken us this far. All advanced care of patients before practicums had been virtual and case studies. I have limited experience in a clinical setting in caring for adults. I agree wholeheartedly with the transition from a registered nurse mindset to a provider’s perspective will be a challenge, providing the opportunity for personal and professional growth and building up confidence as a provider (Vojta et al., (2023).
I also find Medicare and Medicaid billing and coding overwhelming due to no experience whatsoever. My clinical site does incident to billing fees for 100% reimbursement (Bupert, 2020). Although a coding and billing department is available, due to the remote location and decreased access to the department, my preceptor chooses to do his own coding and billing for maximum reimbursement. By week four, I would like to rely less on him in selecting the proper coding for the visit and procedures. As we progress through this course, our knowledge and understanding of these identified challenges will be much improved and carried on to our practicums.

Kristina Suzette Reed The expectations I want to achieve from this course are gaining the necessary knowledge and skills to become proficient and an effective provider. I anticipate gaining a better understanding of enhanced nursing competencies, including assessments and diagnostic procedures, managing chronic conditions, prescribing medications, patient education, and developing a treatment plan properly. I also want to learn more about the ethical and legal considerations of being an advanced provider.
My strengths are critical thinking skills, advanced assessment techniques acquired in my many years of ICU nursing, and communication and empathy for my patients. Providing education to both my patients and their families is another strength I have due to teaching families and patients about how their chronic conditions can lead to long-term effects if not managed properly.
Managing newly diagnosed conditions is definitely a weakness that I have, along with knowing which first-line medications will benefit my patient the best. Another weakness I have is the business side of healthcare. The different reimbursement rates and how charges can be billed will be a learning curve that I must understand to avoid negative impacts on future employers.
The career goals that I believe Walden will help me accomplish are how to be the best provider I can and how to accurately find current evidence-based practice information to apply in the care I give. Obtaining my FNP through Walden will help me establish care for the many rural populations here in Mississippi, which has always been my end goal. Working in the ICU, I see daily the effects of people going without care simply because they have no access or their socioeconomic situations prevent them from obtaining consistent and proper care. One study states that states with less restrictive scope of practice for APRNs have shown significant increases in access to providers by 62%, while states with restrictions at 35%. The same study has shown that APRNs provided the proper care for minor injuries and performed better than novice physicians in reading medical histories and providing proper planning for patient follow-up (Ortiz et al., 2018). According to the Mississippi Department of Health (MSDH), the Mississippi Delta has the poorest health outcomes in the state. The delta not only has the highest loss in years of potential life lost, but it also has the highest percentage of low birth weight in infants. The MSDH also states that the majority of families residing in rural areas are black, with the highest poverty rates in the state and country (Williams & Sprinkle, 2021). Due to this disparity, their needs are often not addressed, leading to long-term effects of chronic illnesses left untreated. This is the area I want to serve, and I believe that with the strong foundation of knowledge I will gain while at Walden, I can provide the appropriate care to these individuals in the Delta and help them lead long lives due to increasing the accessibility to care.

Throughout my career as a registered nurse, I have worked in various nursing settings ranging from Long-Term Care, hospitals, cardiac office settings, Coumadin, and heart failure clinics, and currently working as a case manager for an insurance company. My expectation of this course is to understand specific disease processes and how these different disease processes can be assessed, diagnosed, and care planned to meet the needs of each patient. Studies have shown that direct clinical practice while working with preceptors deepens clinical and psychosocial assessment skills (Schlunegger et al., 2023). Working as a case manager for the past 10 years, I have learned to focus on the positive aspects of patients, families, and their situations, for them to make up for their deficits. Understanding the case management process of intake and needs assessment, care service planning, monitoring, and evaluation to inspire patients to utilize their resources, skills, and goals to overcome challenges and achieve the desired health outcomes is a great background to any disease prevention and management process. With care coordination being central to my current position, it is one of my greatest strengths as I work towards embracing the competencies of comprehensive health assessment, diagnosing, care planning, coaching, and coordination as an Advanced Practice Registered Nurse (APRN). It is through effective comprehensive assessment, coordination, and collaboration with patients, patient families, and other providers that APRNs can build bridges to demonstrate competence in the management of patient illness (Schlunegger et al., 2023). There is a tremendous need to enhance nurses’ collaboration and coordination skills to achieve competency in evidence-based practice to ensure the highest quality of care and best population health outcomes (Melnyk et al., 2018).
Challenges and Nursing Practice Competencies
Although my current position equips me with the skills of being able to assess and collaborate with patients, patient families, and providers as a registered nurse, the main challenge will be to embrace nursing practice competencies as a leader and be able to integrate knowledge, research, and critically analyze patient situations to facilitate patient-centered care for health promotion. I am very curious and hope this class can help improve my fundamental skills to actively listen and identify patient barriers and utilize a holistic approach with diagnostic skills to be able to select appropriate individualized treatment strategies for each patient (Schlunegger et al., 2023).
Since acquiring leadership nursing competencies occurs over time, the integration of coursework and clinical learning in this class seems promising in shaping us to become competent APNs who can assist with continuous quality improvement and providing quality reliable health care (Heinen et al., 2019). It is through continued practice that I would hope to be able to embrace critical and diagnostic reasoning skills in clinical decision-making to help with obtaining a comprehensive physical examination, identifying normal and abnormal findings, and be able to prioritizing patient health needs. Although studies have shown that limited information is available regarding competencies in leadership, ethical decision-making, and evidence-based practice, I hope to get an understanding of these core competencies through mentorship, and practice experience in clinical, and didactic classes.
As for my career goals as an APRN, my heart’s desire has always been patient education on disease prevention and management. Studies have shown that core competencies for APRNs are acquired through mentored patient care experiences including independent and interprofessional practice collaboration (American Association of Colleges of Nursing, 2010). This course will help me accomplish my goals through clinical education, knowledge, and skills, and embracing the scope of practice as an APRN. I am looking forward to polishing my leadership skills while learning from my colleagues, my mentors, preceptors, and my professor’s experiences.

Martha, thank you for sharing your experiences and ambitions for the future. Being a case manager has equipped you with many skills to be successful. You have first-hand experiences with patients’ hardships with insurance companies, coverage, and availability to obtain medications or treatments. Your career path includes experience with teaching and problem-solving. Leadership is an essential competency utilized by a nurse practitioner. You mentioned feeling that it is your weakness; many opportunities for improvement are available during this course.
Leadership can be displayed in two ways. Formal leaders are assigned by their management role, and informal leaders display leadership in patient-focused clinical practice (Lamb et al., 2018). Both positions are leaders. Nurse practitioners are expected to show leadership qualities in their work. We demonstrate this in our clinical practice, and it is often overlooked because it is shown in an informal role. Clinical practice leadership is just as important as a leadership job title and should hold the same weight.
A leader is a person who encourages others to follow in a vision. Sets an example: As a nurse practitioner, you lead your patient to follow healthy patient-centered care. In your current practice, you have been demonstrating skills in leadership by advocating for quality and cost-effective care. Another example has been communicating your knowledge to patients in practice and collaborating with other stakeholders in healthcare, like patients, community resources, and healthcare teams (Nurse Journal, 2022).
There are resources to assist nurse practitioner students with obtaining core competencies during clinical practice. As graduates, we must demonstrate strong knowledge, perform skills, and work autonomously in clinical practice. The ability to do so is developed during a mentored clinical rotation, experience with patients, experience with technology, and Interprofessional practice (National Organization of Nurse Practitioner Faculties, 2022). Good luck with your future career; there is an excellent need for practitioners who can educate and communicate with passion and an understanding of patients.Reference

The healthcare needs of our society are increasing and the need for competent providers who are able to deliver high quality care is crucial. After working in both outpatient and majority inpatient settings, I have seen the increase in patients living with multiple medical conditions which can become a challenge for providers when it comes to managing patients’ care. You shared that as a case manager, your assessment of patients’ needs helps for care service planning by inspiring patients to identify certain components to reach their desired outcomes. I can relate to this because I see how many factors contribute to the outcomes of patients. I agree that it is important to understand a person’s skills and resources in order to create a personalized plan of care that is suitable for that particular patient. When thorough assessment takes place, the provider is able to develop a plan where outcomes are more attainable. Mihaly & Gatewood, 2022, explain how important it is for providers to be strong with history taking and physical examination. These are also a vital part of the assessment process. I can see how NP students who may have not had much of a background with direct patient care could struggle with assessment and identifying patient needs. Clinicals will enhance students’ confidence and skills. Mihaly & Gatewood, 2022, explores how the implementation of a workshop helped NP students with their clinical practice. “The workshop’s overriding objective was to reinforce critical skills for all students and fill in gaps for students. The workshop consisted of a history taking exercise, a low-fidelity simulation experience, and the documentation of the encounter by completion of a SOAP,” (Mihapy & Gatewood, 2022). Results revealed that students’ confidence and competence enhanced greatly. I feel that these types of workshops are beneficial for NP students to enhance areas that they identify as challenging. Dugan & Altmiller, 2023, explain that “competency is not a one and done achievement. Many learning activities involve the layering of the knowledge, skills, and attitudes reflective of NP practice. Simulation can help in mitigating some of the student learning consequences of the healthcare environment. Providing students with authentic case scenarios to build competency can provide opportunity for students to master the most important hallmarks of advanced nursing education.”
Another challenge that NPs can face is in regards to the collaboration with another provider, especially if the NP is practicing in a state with a restricted scope of practice. “A lack of consensual definition and differing expectations of NP’s areas of responsibility on the part of the team members both constitute obstacles. Interprofessional collaboration can be impeded by these uncertainties regarding competencies,” Schlunegger et. al, 2023). I believe that the NP’s confidence and competency is also affected by the other interdisciplinary team members, just as within any professional team. If the team is supported of one another, this will promote a healthy and productive team.

This course is the first opportunity to experience patients in a non-acute clinical setting. I have worked with patients in the hospital’s critical care unit for numerous years. In this course, the expectation is building on my skills, applying knowledge, and working towards a career as a graduate independent nurse practitioner (National Organization of Nurse Practitioner Faculties, 2022). As the course progresses, my confidence in identifying patients’ conditions, diagnoses, and treatment plans will improve. My adult family practice rotation is in a local community office. One Physician in his practice cares for mainly adults 60 and older. The patients have seen him as their provider for many years and developed an incredible, trusting relationship. The strengths of research that I have learned in my previous courses will help me develop my nursing practice competencies. I am also very aware of cultural diversity and the importance of developing mutual respect when including patients in the medical decision-making process. One challenge I face in the rotation is developing technology skills; the clinic is significantly behind in computer technology. Most of the paperwork is done on paper. I have found that he receives labs through the mail, and the patient’s hospitalization records are often delayed. This site does not do any procedures in the office; all procedural skills are consulted outside facilities. I will be able to seek skills for check-off at other rotations.
The National Organization of Nurse Practitioner Faculties (NONPF) developed a guide of nine core competencies necessary for nurse practitioner’s future success following their education into the future. These include Scientific Foundation, Leadership, Quality, Practice Inquiry, Technology and Information Literacy, Policy, Health Delivery System, Ethics, and Independent Practice (Nurse Journal, 2022). We have been trained in our previous classes in many of these competencies; now, I will be including all the skills learned in becoming more independent in my practice. Walden University follows many NONPF recommendations for a successful NP clinical rotation. The program has promoted a solid framework to be successful. The structure eliminates known obstacles between the clinical preceptor, facility, and student (Pitts et al., 2019). My career goals are to become more independent in my knowledge, education of patients, and diagnosis of adult patients seen in a family clinic. I will collaborate with my preceptor to formulate a plan of care that includes patient-centered focus and preferences. I aim to serve clinic patients as a nurse practitioner, including being current on best practices through research, incorporating interdisciplinary consults, and providing education and patient advocacy (American Association of Nurse Practitioners, 2022).

In taking this course, it is my hope that I am able to easily distinguish between the different disorders as I see them in the adult realm. It is my hope that knowing exactly what to look for to assist in patient diagnosis will begin to come to me like the back of my hand. Confidence is key! Being able to know exactly what diagnostics to order and what tests for different disease processes is what I look forward to really delving into with this class. According to Nurse Journal, nurse practitioners are expected to have core competencies to effectively deliver safe and effective care. (Nurse Journal 2023). The adult population is unhealthier as the years are progressing and helping them is my main objective. Being able to work in this setting will assist me in becoming a sufficient FNP. I have a desire to do nurse practitioner, but not a burning desire to care for the pediatric community. With FNP I can care for any person, but most likely will be the adult population. I believe my biggest challenge as far as competency will be knowing exactly which medications can’t be prescribed with another medication without having to reference it. I am also looking forward to what is standard routine practice for certain lab values. Certain lab values stick out to me like a sore thumb, and you know right off what to do for the abnormal lab value. My strength as far as competency I believe is patient safety. National Library of Medicine suggests, competencies for nurse practitioners are a standard to ensure professiona development and best practice. ( Schlunegger MC, et al., 2023). I always will ask questions on anything that I am unsure of. According to Ebbert and Connors, patients are the best indicators for feedback on how comfortable nursing students made them feel based upon their knowledge during their clinical experience. (Ebbert, D., Connors, H., 2004). The patient being safe and receiving the correct care is my main concern. This course will ensure that I will be able to effectively and efficiently care for the adult population. Practicums will be an assistive tool to show you exactly how the patient should be cared for and how to come to a diagnosis and proper treatment.

Great discussion! I also identified increasing self-confidence as one of my goals for this course. As we progress through the program, this course and the subsequent courses will provide the education and competencies that studies have shown will also increase our self-confidence to overcome this challenge (Barnes et al., 2021).
I also couldn’t agree with you more about your statement about medications. According to Taylor et al. (2019), a study of self-assessment of competencies, the lack of knowledge of medications and their efficacy and interactions with other medicines is indicated as one of the top challenges among advanced practice nurse students.
With new evidence-based research and new medications, medication management will always be a lifelong learning need; however, I have great faith that by the end of the course, we will have increased knowledge of medication management and increased self-confidence.

These core competencies are essential in becoming a successful nurse practitioner and ensuring the utmost quality of care and patient outcomes (The National Organization of Nurse Practitioner Faculties, 2012). My expectations for this course are to meet the course objectives through mentorship from my chosen preceptor and increase my knowledge base in caring for adults across their lifespan, developing and integrating these core competencies and applying them to patient care as a family nurse practitioner (FNP).
I have been a registered nurse (RN) since 2003 and have worked in many settings. I have developed a vast knowledge and skill set and consider myself an accomplished nurse. The prerequisites to taking this course are advanced, and although they have prepared me for taking this course, I am trepidious. Research has shown that the stress and unknown in this transitional role are not uncommon and stressful; however, educational preparedness, competency, mentorship, and increased self-confidence all contribute to overcoming this challenge (Barnes et al., 2021).
In self-reflection, one of my greatest strengths is communicating with my patients. Communication is key in all aspects of patient care to meet their needs and improve patient outcomes (Kwame & Petrucka, 2021). I will use this skill daily to connect with patients when obtaining a health history, diagnosing, treating, and patient education. My biggest challenge at this juncture is changing my mindset from caring for patients as an RN to applying new knowledge at a level of care as an FNP, along with FNP competencies.
I view this stage of progress as a stepping stone for transitioning from an RN mindset to a provider’s perspective. This course aims to increase my self-confidence and competencies as a nurse practitioner student while developing a sound knowledge base in caring for adults of all ages to provide optimum care. Moreover, ultimately graduating and passing the FNP certification exam.

Tanya Benita Martin Strengths and Challenges Related to Nursing Practice Competencies
I have strengths and shortcomings in my competencies as an advanced nurse practitioner. Critical thinking, efficient communication, analytical skills, creating trustworthy relationships with my patients and their families, exhibiting empathy, and instructing patients and family members throughout their care and treatment are the stability I bring to this clinical course and to my career. For the treatment and recovery of patients, effective patient-provider communication is essential (Kwame & Petrucka,2021). Nurse practitioners collaborate with patients, physicians, nurses, and other medical professionals (Farrell et al.,2022). They need excellent written and verbal communication skills to successfully clarify diagnostics or test results and go over treatment regimens with patients and their family members. Delivering evidence-based treatment and having strong leadership qualities enhance patient outcomes and lower costs. Throughout the patient’s treatment and care, I should also provide learning opportunities that improve their knowledge and abilities.
Nursing competencies, however, are accompanied by several difficulties, including a nursing shortage, an aging nursing workforce, a pay gap for nurses, rising demand for nursing due to the aging baby boomer population, more excellent patient insight and technology, and changing practice settings. By addressing these issues ethically and keeping an eye on the needs and health of the community, advanced practice nurses can guarantee the community’s demands are met. (Bartel & Buturusis,2020).
Career Objectives
I have established several goals and objectives to advance in my profession as a skilled nurse practitioner. Being able to work in a range of settings and having unique employment prospects make specializing in a family practice area one of my top priorities. By preparing me for complex clinical assessments in a primary care context, this course will assist me in achieving my goal. One of the categories that the nurses assigned to their sources of knowledge in practice is experience-based knowledge. (Acebedo-Urdiales, Medina-Noya, & Ferré-Grau, 2019). This training will help me even more in accurately assessing, diagnosing, and treating patients who require medicine and informing them about how to lead healthy lifestyles that will promote health and ward off illness.
Additionally, I will be able to pick up fresh knowledge that is essential for giving patients evidence-based care. Learning new skills further aided my growth in self-assurance, professional development, and ability to offer my patients the finest care possible. Lastly, this course’s clinical and theoretical components will enable me to better incorporate cultural preferences, values, health beliefs, and behaviors into managing patients in primary care settings.

Lauren Leigh Rivers As a healthcare professional, I have high expectations for the Primary Care Across the Lifespan course. I aim to gain the knowledge and skills necessary to provide comprehensive and continuous care to patients of all ages. The course should cover important topics such as preventive care, chronic disease management, mental health, and treatment plans for adult patients. My ultimate goal is to deliver patient-centered care tailored to each patient’s individual needs. This course is crucial to healthcare education and is vital in improving patient outcomes. These guidelines aim to teach NP students critical and reflective thinking skills so they can apply new knowledge effectively. This course also aims to help students communicate effectively, promote health, prevent illness and injury, and support diversity and multiculturalism to create positive social change in the community.
According to the Institute of Medicine’s (IOM) 2010 Future of Nursing report, novice nurse practitioners (NPs) require a strong educational background and prior work experience to transition into primary care (Faraz, 2019).
My goal for this course as a healthcare professional is to understand health assessment theory and practice thoroughly. This knowledge will enable me to effectively apply it in my profession and ultimately improve the health outcomes of my patients. I will be better equipped to translate my newfound knowledge into practical clinical skills by establishing a strong foundation. One of the strengths that can help me in a successful NP role transition is my prior work clinical experience obtained over the years. However, one of the skills I need to develop is communication. This course would help me overcome this challenge since one of the outcomes listed is to help students communicate effectively, promote health, prevent illness and injury, and support diversity and multiculturalism to create positive social change in the community (Walden University, 2023).

Great post! Graduates of Nurse Practitioner programs possess a wealth of knowledge, skills, and abilities crucial for independent clinical practice. Through mentored patient care experiences, they acquire the essential NP Core Competencies, which include the ability to practice independently and collaboratively with other healthcare professionals, analytical skills for evaluating and providing evidence-based care that is focused on the patient, and advanced knowledge of the healthcare delivery system (The National Organization of Nurse Practitioner Faculties, 2012). Mentored patient care experiences are invaluable in shaping healthcare providers. By working alongside experienced mentors, we can observe firsthand the workings of patient care and gain a deeper understanding of patients’ complex issues. These experiences will demonstrate the importance of active listening, empathy, and collaboration to provide patients with the best care.

Great post! I agree that you wanted to fully comprehend health assessment theory and practice it after taking this course as a healthcare professional. You can use this information professionally to help your patients’ health results. By laying a solid foundation, you will be better prepared to convert your newly acquired knowledge into applicable clinical abilities (Farrell et al., 2022). Gloria, your prior work-related clinical expertise accumulated over the years is one of the skills that can continue to help you successfully transition to an NP function. This is one of my shortcomings, too, because communication is a skill, I’m better at but need to be more effective in. I believe the goal for this course as a healthcare professional is to understand health assessment theory and practice thoroughly. This knowledge will enable us to effectively apply it in the profession and ultimately improve the health outcomes of patients (Kwame & Petrucka, 2021). Since one of the aims of this course is to help students communicate effectively, promote health, prevent illness and injury, and support diversity and multiculturalism to foster good social change in the community, I believe you would be able to overcome this difficulty with the help of this course.

Great read! I agree, effective communication is essential among practitioners and clinicians. The information transferred should be clear and understandable (Saputra, 2021). Providers should always be aware of their patient’s health literacy, cultural competency, and, if any, language barriers (Ratna, 2019). Ineffective communication will impair quality care, delay care, deter patient outcomes, and increase healthcare costs (2019).

Summary of Expectations
I am excited about starting the clinical rotation and developing skills in this course. I expect to become more comfortable with the interview, review of symptoms, and physical assessment process. I expect to sharpen my clinical correlation skills that will facilitate the formulation of an appropriate and accurate diagnosis and develop an appropriate treatment plan. I also expect to sharpen and improve my clinical documentation skills. I expect to be challenged and given opportunities to perform skills and treatments that are usually performed in a primary care setting. I also expect to be very busy this quarter with direct patient care during clinical days, documenting my experiences and patient interactions, studying, and reviewing disease processes. I expect to gain more confidence with working in a primary care setting as the quarter progresses. I hope these experiences will develop my provider mindset or facilitate thinking more like a provider than a bedside nurse.
Explanation of Strengths and Challenges Related to Nursing Practice Competencies When Working with Adults
As I reviewed the nurse practitioner competencies, I have identified my own strengths and challenges related to working with adults. I routinely educate patients about the importance of preventative care and routine or annual tests and care that is needed to detect treatable diseases. I promote, encourage, and counsel patients about being empowered to ask their health providers questions about their health and to make healthy lifestyle choices that can prevent disease or decrease the impact of diseases. I attempt to foster a trusting therapeutic, respectful relationship with patients so that they will feel comfortable and confident in my care and their willingness to disclose health-related concerns or issues. I use therapeutic communication when interacting with patients and provide emotional support as needed when difficult conversations or decisions are being made about their care. I fully disclose information and discuss disease processes with them so that they will understand why they are exhibiting signs and symptoms. I advocate for patients. I have worked on quality improvement projects that effect the quality and the safety of care rendered. Some of the challenges that I face are related to the cost of care. I have some familiarity with global budgeting, insurance reimbursement, and billing based on ICD codes and DRGs. I need more education in this area. I also need to continue to hone my assessment and clinical correlation skills.
Career Goals or Objectives
My career objectives and goals for this course, if achieved, will prepare me to render quality patient care. I want to work in Women’s Health. I want to provide quality care to women and foster a care environment where they will feel like their clinical presentation od concerns will be heard, respected, and taken seriously. The damage caused to population health by gender inequality across the globe is immense and justifies comprehensive actions addressing gender equity in health at all levels (Manandhar et al., 2018). I also want to enhance my clinical assessment skills.
